About the role
Location: Milwaukee metro area - travel between 6 CBRF locations
Schedule: Full-time, salaried | On-call rotation required
Salary: $64,000–$72,000 depending on experience
Reports to: President
Adava Care operates 13 Community-Based Residential Facilities across the Milwaukee metro, Waukesha County, and Fox Valley regions. We are hiring a Regional LPN to serve as the clinical lead across six of our Milwaukee-area buildings.
You will be the clinical resource for six assisted living facilities, rotating between two buildings per day. You will work directly with building administrators and caregiving staff to ensure residents receive safe, consistent, high-quality care.
What you'll do
What You Will Do:
- Conduct resident assessments, including pre-admission evaluations for prospective residents.
- Review and update Individual Service Plans (ISPs) and ensure care delivery aligns with current plans.
- Audit medication administration records, med carts, and narcotic counts across all assigned buildings.
- Monitor resident changes of condition and coordinate appropriate clinical responses.
- Review and verify physician orders and follow up on pharmacy concerns.
- Train and coach caregiving staff on clinical skills, medication administration, and DHS compliance.
- Complete risk assessments and monthly nursing assessments.
- Communicate clinical concerns to building administrators and escalate issues to the President as needed.
- Report any suspected caregiver misconduct, abuse, neglect, or misappropriation immediately.
- Participate in on-call rotation for clinical questions and after-hours resident concerns.
What we need from you
Minimum Requirements:
- Current Wisconsin LPN license (required).
- Minimum one year of experience in geriatric care, assisted living, home health, or a similar setting.
- Familiarity with Wisconsin DHS 83 CBRF regulations or willingness to learn quickly.
- Ability to work independently, manage your own schedule across multiple locations, and exercise clinical judgment without constant supervision.
- Reliable transportation to travel between six Milwaukee-area facilities daily.
- Completion of 15+ hours of continuing education annually as required by the State of Wisconsin.
Preferred But Not Required:
- Experience in a multi-site or regional clinical role.
- Experience with electronic health records, ALIS preferred.
- Prior involvement in state survey preparation or regulatory compliance.
